If your practice would like to get involved and try and make Make a date Mate campaign a success then please fill in a registration form-

Once you have registered you will be sent out a "Make a Date Mate" fundraising kit that contains:

• An A3 poster to advertise the event to your patients
• 10 x Make a date mate buttons for all the staff to wear in November
• 100 "make a date mate" urine collection pots- each with a make a date mate buttons within them-
• 50 Make a date mate stickers to put to extra urine sample pots
• The bank account details of the Prostate Cancer Foundation so that your practice manager can deposit the raised funds directly into the PCF bank account.

The fundraising is easy.

• The staff in November wear a Make a Date Button and ask patients if they would like to make a coin donation to the Prostate Cancer Foundation.
• The patient is handed a urine pot with a "make a date mate sticker on it" and a "make a date mate" button inside.
• The patient takes out the button, hopefully puts it on and puts a coin donation back into the pot and hands it to the medical receptionist/ Practice Manager
• The Practice Manager at the end of November collects all the coin donations and deposits them into the Prostate Cancer Foundation bank account.

Their will be prizes in each state for the Practice Manager's that raise the most funds for the Prostate Cancer Foundation.
